Lockport SC Player Forms

The following forms need to be filled out, signed and turned in as soon as possible. Only fill out the information of players and parents, the club will fill in the Club and team information.

All players must fill out:

1. Lockport Soccer Club waiver

All Boys team players must fill out:

1. IYSA medical waiver
   IYSMedicalReleaseForm.pdf
2. NISL player registration form

3. US Soccer Club player waiver
   https://www.lockportsc.com/content/forms/US-CLUB-Medical-Form-2016-PDF-Fillable-Form.pdf

All Girls teams players must fill out:

1. IYSA medical waiver
   IYSMedicalReleaseForm.pdf
2. IWSL player registration form
   IWSL Player registration Form 2019-20

We will also need copies of your player(s) birth certificates for league registrations and a photo for player passes. The required documents can be sent to director@lockportsc.com.
